Evidence Documents for Higher Specialty Training Application

As part of my application to higher specialty training (HST) in the UK, I have decided to collect all evidence documents and publish them online. I already have them stored on my hard drive but I thought it would be helpful to others who are also interested in applying to HST at a later date.

Text highlighted in green applies to my application, highlighted in yellow/orange is what is possible before I submit my application (such as scoring a full 8/8 for MRCP UK; scoring 8/8 for a publication as first author).

My scores in all areas of the application are highest, except in Additional achievements, for which I can only get 1 point and score for training in teaching is likely to be the same at 2 points.

Scored 3rd amongst a total of 134 graduates who passed. Graduated MD General Medicine with honors.

8. Leadership and Management - 7 month Administrative and Academic Financial Secretary for the Telangana State Senior Resident Doctors Association (TSRDA) from November 2021 to May 2022. 

9. MRCP UK - Status - Cleared Part 1 & Part 2 Written; Awaiting a slot to take PACES. I now have the full MRCP UK diploma.

A 54 year old gentleman presented to my clinic with a chief concern of altered bowel habits since 12 years. A principled and inquisitive man, he reports first having altered bowel habits nearly 14 years ago, manifested by an increase in time spent in the loo for complete evacuation. On pressing further, he says that his visit to the wash is often preceded by a vague pain in his abdomen, which apparently may or may not relieve with defecation. He describes the pain as a poorly localising, diffuse, spasmodic pain that can last from anywhere between 5 to 15 minutes.
